a long-standing history of chronic ethmoidal osteomyelitis and diabetes, was admitted to Dr.Jivraj Mehta Hospital, Ahmedabad, for a polypectomy procedure. He had been dealing with chronic sinus infections for several years, which required surgical intervention. Despite his diabetes, his pre-operative health was stable, and the procedure was planned after thorough evaluations.<\/p><p>During the polypectomy surgery, there were no complications, and the procedure was uneventful. However, post-surgery, he developed pulmonary oedema, which significantly impaired his ability to breathe. Intubation was performed to manage his breathing, and he was placed on a ventilator. The timing of this event was critical as he was still in the window period for IV thrombolysis, a treatment commonly used in acute ischemic strokes. However, because of his recent surgery (within 24 hours), thrombolytic agent could not be administered. This posed a significant challenge, but with prompt involvement of neuro-interventional physician it could be negated promptly.The multidisciplinary team decided to proceed with mechanical thrombectomy. This intervention involves physically removing the clot that causes the stroke, a process requiring specialized care. However, as Mr. Saiyad was being transferred to the catheterization lab for the thrombectomy procedure, he experienced a cardiac arrest. Emergency resuscitation efforts were immediately deployed, and after stabilization, an urgent diagnosis of a fresh anterior wall myocardial infarction (MI) was made.<\/p><p>In light of these findings, the team quickly pivoted to a dual intervention approach. Mr. Saiyad was simultaneously treated for mechanical thrombectomy and percutaneous transluminal coronary angioplasty (PTCA), an essential procedure to clear the blocked coronary arteries and restore blood flow to the heart. On the second day post-thrombectomy, Mr. Saiyad developed an intracranial hemorrhage (ICH), accompanied by increased intracranial pressure (ICP). As a result, craniotomy surgery was performed to relieve the pressure and manage the bleeding. The complexity of his condition required constant monitoring and adjustments in his treatment plan.<\/p><p>Despite these setbacks, the medical team at Dr.Jivraj Mehta Hospital maintained a high level of care. Over the course of the following weeks, with supportive care and continuous monitoring, Mr. Saiyad was gradually weaned off from the ventilator.
